​What is a chronic condition?

A chronic condition is an illness that lasts for at least 6 months.

Many people living with a chronic condition need to manage their condition long-term.

Chronic conditions describe a range of health conditions, including:

  • Chronic (ongoing) and complex conditions

  • Diabetes 

  • Heart Disease

  • COPD/COAD

  • Mental Health 

  • Osteoarthrosis

  • Parkinsons Disease

Chronic conditions can often lead to your health gradually getting worse. They can lower your quality of life and affect your independence.

How are chronic health conditions managed?​

Living with a chronic disease can be challenging, but the nursing team are here to help you manage your condition and improve your quality of life. The Nursing staff and practitioners work closely with you to develop a personalized treatment plan that addresses your unique needs.
